To receive a Certificate of Achievement, an assessment must be completed within 30 days of completing the course: this is a root cause analysis investigation, either real or fictitious (if for example you are a consultant who does not work for a site).

“What was your motivation to join this course? ” Part of my job role is to issue NC’s and help my colleagues close them out in the right manner, this course gives me more knowledge about the different methods i can use to help my colleagues.
