What are the biggest gaps you’ve seen between external audits and actual operational risks?

What are the biggest gaps you’ve seen between external audits and actual operational risks?

Maybe not quite what you are looking for, but it may amuse you!
In an effort to save money, a member of the admin team who had been asked to purchase more pH test strips, took it upon herself (albeit with good intentions) to source a new supplier.
She found an incredible deal and ordered the strips from the new supplier.
The testing officer noticed that the strips all produced the same result and queried this.
The outcome was that the pH strips were not actual pH strips but were used to determine if there was a urine infection in the test sample.
Lesson learned - make sure that all involved understand the remit!!!
