Issue 3 places a stronger emphasis on product safety culture, with new requirements for a documented culture plan, management communication, feedback mechanisms, and performance measurement. Auditors must assess not just documentation, but real evidence of cultural maturity across the organisation.

Which types of indicators (e.g. staff surveys, cross‑team forums, incident reviews) have you found most effective for assessing culture in agents and brokers' environments?
 
I think the best way to check a company’s safety culture is to use a few different methods, not just one.
Staff surveys are a good start, because they show what people think and how they feel about safety. But it also helps to have team meetings or forums where people can talk openly about what’s really happening day to day.
Looking at how incidents are reviewed is another strong sign, if people feel safe to report mistakes and the focus is on learning, not blaming, that shows a healthy culture. You can also notice things like how much staff take part in safety talks or training sessions.
So for me, it’s about combining these things, surveys to hear opinions, forums to get honest discussions, and incident reviews to see how people actually behave.
