What methods have you found most effective for analysing data collected during audits to identify underlying issues?

Good question, Jack. Here's what helps me make sense of audit data:
- Group findings into categories (e.g. hygiene, documentation, training). This makes trends and recurring issues much easier to spot.
- Look for repeat problems. If the same issue keeps appearing, it's usually a sign of a system weakness rather than a one-off mistake.
- Keep asking "why". Using the 5 Whys helps identify the root cause instead of just addressing the symptom.
- Compare sites, departments or shifts, where possible. Significant differences can highlight gaps in processes, training or supervision.
- Present findings clearly. The report should be easy to understand, even for someone who wasn't involved in the audit.
