When reviewing your Corrective and Preventive Action records, don’t stop at recording that the corrective action has been completed and verified.
Clause 3.9.1 also requires verification that the correction has been implemented and is effective.
It is quite easy to have a CAPA log showing:
  1. corrective action completed,
  2. verification completed,
  3. verification date,
but no clear evidence that the action was actually effective in preventing the issue from recurring.
A simple “Effectiveness Verified – Yes/No”, together with the verification date and, where appropriate, a short comment or evidence, can make this much clearer.

A small detail, but definitely worth checking when reviewing your CAPA system before an audit.
